Chronic Disease Prevention and Management efforts are dedicated to improving the health and wellbeing of Niagara residents through coordinated approaches to chronic disease prevention, early intervention, and ongoing management. Recognizing the significant impact that chronic diseases have on individuals, families, and the health care system, the group focuses on strengthening supports and improving care experiences for people living with chronic conditions, particularly chronic obstructive pulmonary disease (COPD) and diabetes.
Our Ontario Health Team partners, health care providers, community organizations, and system leaders have come together since October 2025 to collaborate on initiatives to improve prevention, care coordination, and disease management across Niagara. Through a person-centered and integrated care approach, the group works to identify gaps, develop pathways, and strengthen partnerships across sectors.

The group collaborates with health care providers, community organizations, and public health partners to identify barriers to screening and implement strategies to address them. By raising awareness about the importance of cancer screening, facilitating access to services, and promoting culturally sensitive approaches, the taskforce aims to ensure equitable access for all Niagara residents, including underserved and equity-deserving populations.
Through its efforts, the Cancer Screening Task Force seeks to empower individuals to take proactive steps in managing their health while fostering a more responsive and inclusive health care system in Niagara.
