Inside, you’ll find stories of partnership and impact—from thousands of Niagara residents connected to team-based care, to the continued growth of SCOPE Niagara and online appointment booking, to new palliative care pathways, grief and bereavement supports, remote care monitoring, dementia care initiatives, and the launch of a Long-Term Care Advisory Council. The report also celebrates the essential role of Patient/Client and Family/Caregiver Advisors, whose lived experience continues to shape more inclusive, person- and family-centred planning across the NOHT-ÉSON.
