The Dementia Care Working Group (DCWG) brings together health care providers, community organizations, caregivers, and individuals with lived experience to improve the quality of dementia care across the Niagara region. Through collaboration, knowledge sharing, and quality improvement initiatives, the group works to identify gaps in care, enhance support for individuals living with dementia and their caregivers, and promote coordinated, person-centred care.
Guided by provincial best practices and dementia care standards, the DCWG supports the development of innovative, equitable, and community-based approaches that improve experiences and outcomes for those affected by dementia throughout their care journey.
