The Stroke Care Working Group (SCWG) was established in May 2021 to address the needs of individuals recovering from strokes. The NOHT-ÉSON identified people post-stroke as a priority population, underscoring the importance of improving outcomes for this group.
The Stroke Care Working Group includes representatives from across the stroke care continuum, along with subject matter experts and stroke survivors/caregivers. Its primary purpose is to support and enhance an integrated continuum of care for stroke recovery, ensuring that services are guided by evidence-based practices.
By applying a population health framework and quality improvement processes, the group aims to identify and address gaps in care, ultimately enhancing the quality of life for individuals post-stroke and driving better health outcomes across Niagara.
