In collaboration with the NOHT-ÉSON, the Diversity Institute (DI) has developed a comprehensive Health Equity Toolkit to support over 45 organizations within the NOHT-ÉSON network in advancing their health equity efforts. The toolkit is designed to guide these organizations on their journey to promote health equity for designated populations, including Francophones, Indigenous Peoples, and other equity-deserving groups, in alignment with the Ontario Health Team (OHT) mandate.
The toolkit serves as a strategic roadmap for organizations to understand health equity opportunities, identify key priorities, and implement best practices. It aims to help organizations achieve their goals while fulfilling the broader mandate to improve health equity across Niagara.
The key components of the toolkit are as follows:
- Health Equity Report: Provides in-depth insights and analysis, complemented by four Appendices.
- Health Equity Assessment Tool: A structured tool for evaluating health equity practices and outcomes.
- Health Equity Progress Tracker: Tracks organizational progress in implementing health equity initiatives.
- Health Equity Resources and Best Practices Guide: Offers actionable resources and best practices to support ongoing efforts.
