RISE: Rapid Improvement Support and Exchange

From the site: RISE’s mission is to contribute to the ministry’s OHT Central Program of Supports by providing timely and responsive access to Ontario-based ‘rapid-learning and improvement’ assets. In fulfilling this mission, RISE is committed to ensuring that OHTs have equitable access to support and that members of the RISE community of practice have equitable opportunities to contribute to this support.

Alliance for Healthier Communities

Alliance members serve diverse communities across the province, and they are rooted in the communities they serve. They share a commitment to advancing health equity through the delivery of comprehensive primary health care.”

Ontario Health Teams – Ontario Ministry of Health

From the site: “Ontario Health Teams are being introduced to provide a new way of organizing and delivering care that is more connected to patients in their local communities. Under Ontario Health Teams, health care providers (including hospitals, doctors and home and community care providers) work as one coordinated team – no matter where they provide care.”
