NOHT-\u00c9SON is well on its way towards creating a seamless system of connections among health care agencies (right) through policy conversations, resource alignment and technological innovations which provide better outcomes for everyone.<\/em><\/p>\n<\/div><\/section><\/div>\n<div  class='flex_column av-25ne9r-de5231bb2611f7bc5429dee4fd54e725 av_one_full  avia-builder-el-16  el_after_av_one_full  el_before_av_button  first flex_column_div  column-top-margin'     ><p><section  class='av_textblock_section av-l0snmbr3-f43c2fb54dad023d3003f79d06669fec '   itemscope=\"itemscope\" itemtype=\"https:\/\/schema.org\/BlogPosting\" itemprop=\"blogPost\" ><div class='avia_textblock'  itemprop=\"text\" ><p>Carolyn says working in silos\u2014organizations or even sectors operating as stand-alone entities disconnected from related enterprises\u2014has been the standard in health care for too long. Programs like the one Mary enjoyed are not unique. Organizations create them when necessity, capacity, timing and funding opportunities coincide. Challenges arise, however when these factors are not aligned: services are not always available to everyone who needs them.<br \/>\n\u201cA fresh approach that breaks traditional communication barriers and facilitates knowledge exchange is an idea whose time has come,\u201d Carolyn says explaining her decision to join NOHT-\u00c9SON at the first available opportunity. \u201cGetting to know professionals in other health care sectors broadens my perspective and creates new possibilities for change.\u201d<\/p>\n<p>Carolyn\u2019s opportunity came sooner than she expected.<\/p>\n<p>At one of the early NOHT-\u00c9SON Planning Table meetings, a group of primary care professionals were invited to discuss a Briefing Note they had submitted: administering vaccines through family doctors and pharmacists had become unsafe and the logistics of arranging some Pop-up and Drive thru Vaccine and Flu clinics for the fall of 2020 had become challenging. The issue was clear. Primary care had no capacity to transport the most vulnerable people to vaccination sites.<\/p>\n<p>As soon as the floor opened up for discussion, Carolyn spoke up. \u201cThis is how the community support sector can help,\u201d she said. \u201cWe have enough lead time before implementation to increase our capacity to support vulnerable individuals for whom self transportation, public transportation or reliance on family, friends or caregivers is not an option.\u201d<\/p>\n<p>Recognizing the value of Carolyn\u2019s idea, the Planning Table erupted (as only ZOOM meetings can) in congratulatory mouse-clicks of applause, nods of approval and verbal pledges of support in solidifying the details. Most importantly, the variety of voices from across the health sector&#8211;long term care, addictions and mental health, home care, primary care, emergency services, to name a few\u2014meant that issues of significance across the health system could be meaningfully discussed and decisions could be made efficiently.<\/p>\n<p>Answers began to emerge at the regular ZOOM meetings of NOHT-\u00c9SON\u2019s Planning Table. The tone of these meetings was already unique. Traditional formalities of \u2018rules of order\u2019 were replaced by casual conversation etiquette and simple civility. Participation was equally open to everyone. And decisions were made by consensus: no voting. Issues were discussed until compromises\u2014solutions everyone could live with&#8211;were reached.<\/p>\n<p>Another moment of cross-sector collaboration at NOHT-\u00c9SON was born, CSSN\u2019s Covid-19 transportation program gradually came to life.<\/p>\n<p>\u201cFor me, it was the first big \u2018aha moment\u2019 that erupted at our NOHT-\u00c9SON Planning Table Meetings,\u201d says Carolyn describing the moment the proverbial penny dropped, underlining to the group the value of connection and collaboration. \u201cIt marked a major step forward for Niagara\u2019s community services sector in delivering clear benefits to the people we are privileged to serve.\u201d<\/p>\n<p>Carolyn is now regularly invited to participate in cross-sectoral conversations involving mental health, primary care, home care, palliative care and many other aspects of health policy development.<\/p>\n<p>\u201cNOHT-\u00c9SON\u2019s connection and collaboration strategy is working,\u201d Carolyn says. \u201cIt offers clear advantages for building a more effective health system, stronger communities and improved population health.\u201d<\/p>\n<\/div><\/section><br \/>\n\n<style type=\"text\/css\" data-created_by=\"avia_inline_auto\" id=\"style-css-av-l0snniqr-f7e75c4d6486a6d0298f519cbafbaae2\">\n#top .hr.av-l0snniqr-f7e75c4d6486a6d0298f519cbafbaae2{\nmargin-top:30px;\nmargin-bottom:30px;\n}\n.hr.av-l0snniqr-f7e75c4d6486a6d0298f519cbafbaae2 .hr-inner{\nwidth:200px;\nborder-color:#00a0df;\n}\n<\/style>\n<div  class='hr av-l0snniqr-f7e75c4d6486a6d0298f519cbafbaae2 hr-custom  avia-builder-el-18  el_after_av_textblock  el_before_av_textblock  hr-center hr-icon-no'><span class='hr-inner inner-border-av-border-fat'><span class=\"hr-inner-style\"><\/span><\/span><\/div><br \/>\n<section  class='av_textblock_section av-l0snmza1-70235aab56d5bdbc517a3484155d7e0c '   itemscope=\"itemscope\" itemtype=\"https:\/\/schema.org\/BlogPosting\" itemprop=\"blogPost\" ><div class='avia_textblock'  itemprop=\"text\" ><p>Meanwhile, Mary\u2019s trips to the hospital are now fewer and farther between. Her groceries are delivered regularly, she uses transportation services for daily routine travel and enjoys the rapport she built with Alesha.<\/p>\n<p>\u201cTalking about my experience with the COVID19 transportation service is a pleasure,\u201d Mary says. \u201cIt was a major benefit to me and the quality of service was high,\u201d Mary says.
