sont admissibles au NOHT-\u00c9SON :<\/p>\n<ul>\n<li>Soins primaires (y compris les soins primaires interprofessionnels et les m\u00e9decins)<\/li>\n<li>Soins secondaires (c\u2019est-\u00e0-dire, services m\u00e9dicaux et chirurgicaux en milieu hospitalier et ambulatoire, y compris services de sp\u00e9cialistes)<\/li>\n<li>Soins de sant\u00e9 \u00e0 domicile<\/li>\n<li>Services de soutien communautaire<\/li>\n<li>Services de sant\u00e9 mentale et de lutte contre les d\u00e9pendances<\/li>\n<li>Services de promotion de la sant\u00e9 et de pr\u00e9vention des maladies<\/li>\n<li>R\u00e9adaptation et soins complexes<\/li>\n<li>Soins palliatifs (c\u2019est-\u00e0-dire, h\u00f4pital)<\/li>\n<li>Soins en \u00e9tablissement et soins de transition \u00e0 court terme (c\u2019est-\u00e0-dire, logements avec services de soutien, foyers de soins de longue dur\u00e9e, maisons de retraite)<\/li>\n<li>Placement en foyer de soins de longue Aussi, les statistiques recueillies aupr\u00e8s des fournisseurs de soins de sant\u00e9 de Grimsby\u00a0et\u00a0de\u00a0West\u00a0Lincoln\u00a0sont\u00a0rapport\u00e9es\u00a0par\u00a0l\u2019\u00e9quipe\u00a0Sant\u00e9\u00a0Ontario\u00a0de\u00a0Hamilton.<\/p>\n<h3>Acc\u00e8s aux services de soins de sant\u00e9 \u00e0 Grimsby et West Lincoln<\/h3>\n<p>M\u00eame si les fournisseurs de soins de sant\u00e9 de Grimsby et de West Lincoln sont membres de l\u2019\u00e9quipe Sant\u00e9 Ontario de Hamilton, les r\u00e9sidents de Niagara, y compris les r\u00e9sidents de Grimsby et de West Lincoln, peuvent utiliser tous les services de sant\u00e9 dans la communaut\u00e9 de leur choix.
