Cette nouvelle solution de sant\u00e9 num\u00e9rique offre aux patients un point d\u2019acc\u00e8s unique pour consulter leurs dossiers m\u00e9dicaux provenant des h\u00f4pitaux participants.<\/p>\n<p>La solution ConnectMyHealth peut \u00eatre utilis\u00e9e sur des ordinateurs de bureau et des appareils mobiles et son utilisation est gratuite. La plateforme est diff\u00e9rente des autres portails de patients, car elle regroupe en une seule vue consolid\u00e9e les diff\u00e9rents dossiers m\u00e9dicaux cr\u00e9\u00e9s dans les h\u00f4pitaux du sud\u2011ouest de l\u2019Ontario que les patients ont visit\u00e9s. Les h\u00f4pitaux participants comprennent notamment Sant\u00e9\u00a0Niagara et le Centre de sant\u00e9 et de r\u00e9adaptation H\u00f4tel\u2011Dieu\u2011Shaver.<\/p>\n<p>Parmi les principales fonctionnalit\u00e9s de ConnectMyHealth, notons les alertes par courriel lorsque de nouveaux r\u00e9sultats sont disponibles, un tableau de bord personnalisable et la possibilit\u00e9 d\u2019afficher un calendrier des prochaines visites \u00e0 l\u2019h\u00f4pital.<\/p>\n<p>Les patients peuvent consulter leurs renseignements m\u00e9dicaux \u00e9lectroniques, recueillis \u00e0 partir de divers syst\u00e8mes sources dans la r\u00e9gion du sud\u2011ouest de l\u2019Ontario, notamment\u00a0:<\/p>\n<ul>\n<li>les rapports de radiologie et les images correspondantes (le cas \u00e9ch\u00e9ant);<\/li>\n<li>les rapports et les images relatifs \u00e0 la cardiologie (le cas \u00e9ch\u00e9ant);<\/li>\n<li>les r\u00e9sultats des tests de laboratoire;<\/li>\n<li>les rapports cliniques, comme les sommaires de cong\u00e9 d\u2019h\u00f4pital;<\/li>\n<li>des renseignements sur des m\u00e9dicaments;<\/li>\n<li>les allergies document\u00e9es pendant les visites \u00e0 l\u2019h\u00f4pital;<\/li>\n<li>les rapports de pathologie et de g\u00e9n\u00e9tique;<\/li>\n<li>les d\u00e9tails des visites pr\u00e9c\u00e9dentes et \u00e0 venir \u00e0 l\u2019h\u00f4pital (le cas
