DESCRIPTION:The journey continues for persons living with dementia and their care partners following the move to long-term care. Families often face a significant period of adjustment as their loved one settles into a care facility.\nJoin us as we celebrate the Alzheimer Society Niagara Region’s receipt of an Ontario Trillium Foundation Grant to support the Long-Term Care Transitional Support Service.
